Acanthosis Nigricans Occurring in Association with Adenocarcinoma of the Stomach

Abstract
Clinically acanthosis nigricans can be devided into three types;
1) Pseudoacanthosis nigricans.
2) Benign acanthosis nigricans.
3) Malignant acanthosis nigricans.
Acanthosis nigricans was first described by Pollitzer in 1890. It is a disease of the skin which is characterized by areas of hyperpigmentation and papillary hypertrophy. The lesions are usually distributed and have a number of sites of predilection.
The axillae, neck, external genitalia, groins, face, inner aspect of the tighs, flexor surface of-the elbows, knees, umbilicus and perianal area are the regions most frequently involved.
Histopathologically the lesions are characterized by absolute and relative hyperkeratosis, acanthosis and hyperpigmentation of the basal layer.
Because of its relatively infrequency, difficulty of clinical differential diagnosis from 3 different forms and interesting pathogenesis, one case of malignant acanthosis nigricans is reported.
A 33 year-old man had had gastric, malignancy and subtotal gastrectomy with gastrojejunostomy two years ago.
Postoperatively he had been in good health for a while, but one year later he noted hyperpigmentation on the flexual areas, especially axillae, groins, side of neck, anogenital area, palms and soles, and reddish papillary hypertrophied lesions on the both lips, buccal mucosa and both eye lids.
Under the clinical impression of acanthosis nigricans skin biopsy and explolaparatomy had been performed. The skin showed classical picture of acanthosis nigricans and widcspred interaabdominal adenocarcinoma was found. After three months he was readmitted and was treated with bleomycin for 2 months.
Condition of the skin had improved greatly during the treatment, but expired by widespread internal malignancy.
